How to Register
You may register for the program 3 ways:
1. Register Online.

2. Print a registration form and sent it to the CARA office.

You may send your printed registration form:
By fax – If paying by credit card, you may fax your completed form to us at 312.781.1736.
By mail – Mail your completed form and payment to CARA, 549 West Randolph Street, Suite 704, Chicago, IL 60661. If paying by check, make it payable to "CARA". Do not send cash. Allow plenty of time for your registration to reach us.
3. On Site – you can register at the Super Clinic, or at any of the Packet Pick Up Locations.
